<p>The protein-energy malnutrition or PEM is a condition that includes marasmus and kwashiorkor. Marasmus is a condition in which dry form with extreme emaciation occurs and kwashiorkor usually occurs in children. The marasmus indicates the deficiency of proteins in the body along with the calorie deficiency.</p>
<p><span id="more-170"></span>Marasmus results in severe retardation in the growth. Kwashiorkor occurs in children due to the protein deficiency. But, the protein deficiency is not the sole cause for the occurrence of edema in kwashiorkor. Kwashiorkor can also result in the insufficient or poor quality of the proteins, pregnancy, gastrointestinal loss or tissue injury. The protein deficiency is also noted in adults.</p>
<p>Vitamin A deficiency is also severe in about 30 % of the children suffering from kwashiorkor. They also exhibit xerosis, bitot’s spot and keratomalacia. This deficiency of the vitamin A is not because of the malabsorption but of the intake. For this vitamin supplement are administered orally. The best way to avoid these deficiencies is to consume a high protein diet.</p>
<h3><strong>Dietetic management for PEM</strong></h3>
<p>Kwashiorkor is a disease that is primarily occurring due to the protein deficiency and hence the best way to compensate can only be with the diet. The dietetic management for PEM is framed in such a way that it should fulfill the protein and calorie needs and should eradicate any infection that is present in the body.</p>
<p>For fulfilling the need of proteins, adequate milk and milk products are necessary as this helps a lot in the prevention of kwashiorkor in children and in infants. Whole milk is also not tolerated so, skimmed milk or milk treated with lactose is given to prevent diarrhea. The protein diet that supplies 20% of the calorie is usually favored. The recommended amount of Iodine intake with other vitamins is 150 micrograms per day, for the proper functioning of thyroid hormones- essential for the brain development in fetus.</p>
<p>The research also viewed the nature of Iodine ingredients, whether it is from natural sources as kelp or from salt like Potassium Iodide. It is also noticeable as the lower amount put the unborn on risk, the higher percentage can also cause greater damages, especially in the women who already face thyroid problems.</p>
<p>The food supplements which have the Iodine in the form of kelp contain more than enough Iodine while those with Potassium Iodide have an average level of iodine.</p>
<p>So it is suggested, on the base of this study, women should take Potassium Iodide containing prenatal multivitamins rather than those with kelp.</p>
<p>The researchers, however, urged the responsible authorities to notice this problem because there is no law that requires a vitamin manufacturer to include iodine in prenatal vitamins.</p>

On an average, it stores about 40 to 50 ml of bile. For a normal healthy person, the liver secretes about 700 ml of bile each day. When the secreted bile is sufficient, then the excess bile is concentrated and is stored in the gall bladder.</p>
<p>The composition of the bile includes 40% cholic acid, 40% chenodeoxycholic acid, 2% lithocoholic acid and 18% deoxycholic acid. The occurrence of gallstone is very high in western countries when compared to the Asian countries. These stones when occurring usually do not show any symptom.</p>
<p><span id="more-157"></span>There are different types of gallstones. The biliary sludge occurs as a result of complicated intravenous feeding. The cause of this is due to the fact that the gall bladder does not contract in the absence of fat in the feeds. This process ultimately leads to the stagnation of the bile, then leading to precipitation of the sludge and hence to the formation of gallstone. The gallstones formed in the gall bladder may also contain bile pigments and calcium carbonate.</p>
<p>At times a pure stone of cholesterol can also occur. In normal conditions, the cholesterol, bile salts and other phospholiquids help in keeping the bile in liquid state. When the cholesterol precipitates in the bile, then gallstones are formed.</p>
<p>The precipitation of the cholesterol can occur due to the following reasons. The cholesterol precipitation can occur due to the increased concentration of cholesterol. In patients with the gallstone, the liver produces super saturated bile and the high calorie intake leads to further favors gallstones. The decreased concentration of the bile salts and phospholiquids can also lead to the gallstone formation.</p>
<h3><strong>Dietetic management for gallstone bladder</strong></h3>
<p>The gallstone usually occurs when there is a large intake of calorie in proportion of their protein intake. Therefore, minimum amounts of calorie should be taken in order to maintain a good health. Diets that are protein rich can also increase the concentration of the biliary cholesterol. For a normal diet, 60 to 80 g of proteins is sufficient.</p>
<p>The fats can be consumed as normal and should not be stopped mistakenly. There is no evidence that a low fat diet has any effect on the gallstone treatment and hence should not be avoided. The Chinese foods like fried rice and fried noodles should be avoided completely if they produce any discomfort. In most cases the diet is provided in such a way that the patient receives the normal amount of calorie with proportion to the proteins. Vitamins and other nutrients can be administered in the form of supplements.</p>
